[MY] AM50 Health Screening Submission & Health Champion Medal Redemption – How It Works

Learn how to submit your pre- and post-health screening details, how eligibility is assessed by the system, and how to redeem your Health Champion medal if you qualify.

 

Campaign Overview

This phase allows participants who have completed their health screenings to submit both pre- and post-screening details together for system validation.
Health Champion Medal eligibility is outcome-based and is determined automatically by the system upon submission.

 

Who Can Participate

This phase applies to participants who:

  • Have purchased and completed the AM50 Health Screening Pass (HSP)
  • Have both pre- and post-health screening results available
  • Submit within the campaign period when submission is open

Submission is opened in a later campaign phase (from March).

 

What You Need to Prepare

Before submission, please ensure you have:

  • Your official health screening report
  • Pre- and post-health screening photos (full body)
  • All required health metrics from your report

All fields listed below are mandatory.

 

How to Submit Pre & Post Health Screening Details

  1. Access the AM50 campaign page when submission is open
     
  2. Enter your details:

    o    Name
    o    Email
    o    Preferred medal redemption store
     

  3. Upload photos:

    o    Full body front view
    o    Full body side view
          (Photos are used for verification only and will not be displayed publicly.)
     

  4. Enter health metrics exactly as shown in your official report:

    o    Body Weight (kg)
    o    Body Fat
    o    Muscle Mass
    o    Total Cholesterol
    o    LDL Cholesterol
    o    HbA1c
    o    Submit your pre- and post-health screening details together

    Partial or single submissions are not accepted

 

System Validation & Eligibility

How eligibility is determined

  • Submissions are validated by the system
  • Users will see a result of “Eligible” or “Not Eligible” only
  • Only users with positive improvement outcomes of any one of the health metrics above will qualify for medal redemption

Resubmission policy

  • If you are not eligible on your first submission, you are allowed one (1) resubmission only
  • Resubmission must be made within the campaign period
  • Results from the resubmission will be final

 

Medal Redemption

If you are eligible

  • You will receive an email confirmation with:

    o Your eligibility status

    o The store location selected during submission

    o Instructions for redemption

     

How to redeem in-store

  1. Visit the selected store stated in your confirmation
  2. Show the confirmation screen (click from your email) to Amway staff
  3. Staff will validate your redemption and proceed accordingly

Store selection cannot be changed after submission.
Medal redemption is one-time only per eligible participant.

  • All submission fields are mandatory
  • Submitted information must match your official health screening report
  • Photos are required for verification purposes only
  • Medal redemption is not guaranteed and depends on submission outcome
  • Amway reserves the right to amend the Terms & Conditions without prior notice

FAQs

  • Can I submit only my pre- or post-health screening details?
    ╰┈➤ No. Both pre- and post-screening details must be submitted together.
     
  • Will I see my improvement score?
    ╰┈➤ No. You will only see whether you are eligible or not eligible for medal redemption.
     
  • Can I resubmit my details?
    ╰┈➤ You may resubmit once if you did not qualify on your first attempt, within the campaign period.
     
  • Can I change my selected redemption store?
    ╰┈➤ No. Store selection cannot be changed after submission.
